This form is for the leaders of social walks to print off and use to gather the necessary details from walkers before setting off.  After the walk has finished please give it to the walks secretary if present, or if not email her/him the names and number/email addresses, NOT the emergency contact details. Then please DESTROY the sheet, eg shred it, to maintain data protection.
